Housing Case Manager, St. John’s (Full Time/Permanent)

Summary:
Under the supervision of the Residential Manager, the Housing Case Manager provides case management and housing support services to those individuals who are experiencing homelessness or those who are at risk. Through the use of a Housing First framework, works collaboratively to reduce homelessness and improve housing stability.
Responsibilities:
- Delivers evidence-based interventions to the participants to enhance their daily living.
- Performs regular in-house visits with participants.
- Performs case management with participants having complex needs and barriers, to support housing retention and community stability.
- Completes file reviews, assessments and reports as required.
- Prepares, documents, implements and monitors personalized support plans including short-and long-term goals related to housing stability.
- Identifies problem areas and intervenes effectively in conflictual and crisis situations.
- Maintains collaboration with End Homelessness’ Coordinated Access to Homes (CAH), John Howard Society programs, government and partner agencies for the purpose of case management, alternative program referral and client advocacy.
- Liaise with participant’s external network of supports.
- Engages with landlords to source, secure and sustain affordable housing for CAH participants.
- Develops and presents information sessions to participants and community agencies.
- Attends program related meetings as required.
- Maintains accurate electronic and manual client records and a process for statistical data collection.
- Provides transportation to participants.
- Contributes to the orientation/training of staff/interns/volunteers.
- Engages in professional development as required.
- Ensures confidentiality guidelines are met regarding staff and participants’ information.
- Shares responsibility for maintaining a safe, secure environment for both staff and participants.
- Performs other related duties as required by the Residential Manager.
Qualifications:
- Graduation with an accredited Bachelor of Social Work Degree, Psychology, Counselling Psychology or related field. Some combination of education and experience may be considered.
- Demonstrated knowledge of and a minimum of 3-years experience working vulnerable populations in non-profit, housing or homelessness sector.
- In-depth understanding of government and community resources including income support, benefits and rental supplements.
- Knowledge and understanding of Residential Tenancies Act is an asset
- Experience with and a demonstrated understanding of the offender population and others with complex needs.
- Broad understanding of the criminal justice system.
- Experience in employing Trauma Informed and Harm Reduction approach’s in working with vulnerable population.
- Excellent written and oral communication skills.
- Excellent interpersonal skills.
- Valid driver’s license and access to safe reliable vehicle.
- Proven competency in computer software, experience with HIFIS an asset
- Ability to work independently and as part of a team.
- Multilingual skills and multicultural competence are an asset.
- Certificate of conduct.
- First Aid/MHFA/ASIST Certifications are an asset.
- Enhanced reliability check as required.

Salary: $33.64/hour as per collective agreement; 70 hours bi-weekly,
