Intensive Case Manager Team Lead

Summary:
Under the supervision of the Manager of the Learning Resource Program, the Intensive Case Manager Team Lead provides ongoing support to Case Managers and Community Support Workers. This position provides in-reach at Her Majesty’s Penitentiary (HMP) as well as provides community support services to persons involved with the criminal justice system who are living with a severe and/or persistent mental illness or concurrent disorder. Using a trauma informed approach, the Intensive Case Manager will assist participants develop comprehensive support plans to improve community functioning and quality of life.
Responsibilities:
- Delivers evidence-based programs in a group and/or individual settings in the community and institutions.
- Provides guidance and ongoing clinical consultation support to staff team for purpose of fostering adherence to professional standards.
- Organization and management of program statistics/database including quarterly and annual statistics reporting.
- Review referrals for program eligibility and assigns participants for intakes and supports.
- Ensures staff are implementing current standards and best practices governing program integrity.
- Provides in-reach to individuals incarcerated at HMP, for the purpose of assessment, release planning and mental health counselling.
- Works with the participant to develop an individualized community plan to assist them moving from intensive case management to more independent living.
- Prepares, documents, implements, and monitors personalized support plans including short- and long-term goals related to community reintegration in accordance with professional standards of practice.
- Collaborates with the Community Support Worker, other JHS services, as well as other community and mental health services to ensure that individual support plans are implemented.
- Identifies problem areas and intervenes effectively in conflictual and crisis situations.
- Contributes to an environment that fosters positive mental health for participants.
- Conducts case management meetings with other JHS programs, community, and government organizations.
- Develops and presents information sessions to participants and community agencies.
- Maintains accurate electronic and manual participant records and a process for statistical data collection.
- Assists with research and implementation of evidence-based practices and approaches for ongoing program delivery.
- Provides transportation to participants.
- Meets participants off site.
- Contributes to the orientation/training of staff/interns/volunteers.
- Ensures confidentiality guidelines are met regarding staff and participants’ information.
- Shares responsibility for maintaining a safe, secure environment for both staff and participants.
- Engages in professional development as required.
- Performs other related duties as required by the Learning Resource Manager.
Qualifications:
- Completion of Master’s Degree in Social Work or Psychology.
- Eligibility for registration with a professional body (NLCSW or CCPA).
- Knowledge of clinical practice and standards as per professional body.
- Demonstrated knowledge and understanding of mental health and addictions.
- Demonstrates knowledge of and a minimum 5-years experience working in intensive case management.
- Demonstrated knowledge of Trauma Informed Practice, strengths-focused and solution-focused therapy modalities.
- In-depth understanding of government and community resources.
- Experience with and a demonstrated understanding of the individuals involved in the criminal justice system as well as severe/persistent mental illness or concurrent disorders.
- Understanding of the criminal justice system.
- Excellent written and oral communication skills.
- Excellent interpersonal skills.
- Valid driver’s license and access to safe, reliable vehicle.
- Proven competency in computer software.
- Ability to work independently and as part of a team.
- Multilingual skills and multicultural competence are an asset.
- Certificate of conduct.
- First Aid/MHFA/ASSIST Certifications are an asset.

SALARY: $37.66/hour as per collective agreement; 35 hours per week.
